To apply for Citizenship, you will need to be a legal permanent resident for at least five years and bring the following information:
  • Alien card (mica), Social Security Card, State ID or Drivers License
  • List of travels outside the United States since becoming a legal Permanent Resident. Including month, day, year and place of travel
  • List of where you have lived and worked for the past five years, including address, month and year
  • Information about your spouse including name, date of birth, social security number, alien number and/or date of naturalization (if applicable)
  • Information about your children including address, name, date of birth, and alien number (if applicable)
  • Information about previous marriages (if applicable): date or prior marriage, date the marriage ended and reason marriage ended
  • If applicable, information about any arrests: reason for the arrest, date, place and disposition
  • For men only:  Selective Service number and date of registration
  • A money order for $675.00 made out to the U.S.C.I.S. and $10.00 for photos
  • To replace your Legal Permanent Card, you need the following: 

    • State ID or Drivers Licenses
    • Social Security #
    • Legal Permanent Resident Card
    • Money Order for $370 made out to the U.S.C.I.S

1. Ser residente permanente legal por los últimos cinco años
2. Tarjeta de residente permanente, numeró de seguro social, identificación del estado o licencia de conducir
3. Lista de todos los viajes fuera del país desde que se hizo residente permanente, incluyendo el mes, día, año y el país de destino por cada viaje
4. Lista de sus domicilios e información de empleos en los últimos cinco años
5. Información sobre su cónyuge: nombre completo, fecha de nacimiento, numero de seguro social, numero de mica o fecha y lugar de naturalización (si es aplicable)
6. Información sobre todos sus hijos/as: nombre completo, fecha de nacimiento, numero de seguro social, numero de mica o fecha y lugar de naturalización (si es aplicable)
7. La información suya y de su cónyuge sobre matrimonies previos:  nombres completos, fechas de matrimonio y fechas en que terminaron los matrimoniós, y los motivos por el cual termino el matrimonio
8. Si ha sido arrestado: razón por el arresto, fecha, lugar y resultado
9. Para hombres solamente: fecha y numero de registro con el servicio selectivo
10.  Un giro de $675.00 hecho al U.S.C.I.S. y $10.00 para sus fotos.

Immigration Casework

In Congress, I am the chairman of the Democratic Caucus Task Force on Immigration. In this role, I have helped to prevent the break-up of immigrants' families and have championed initiatives to eliminate the massive backlog of citizenship applications.  Throughout the years I have remained an active leader in the fight to defend the rights of immigrants so they can receive the protections and benefits given to them under the law.

My staff and I have led the fight to help immigrants with a wide range of concerns facing newcomers to our country.  In Chicago, my office has held citizenship workshops, helping over 30,000 people take the first steps towards citizenship. If you have questions about any aspects of becoming a citizen or other immigration concerns, please contact my offices at any time.

Pursuant to the Privacy Act of 1974, I must have a signed  case authorization form before I can begin work on your case. This form is Casework Authorization Sheet is made available for your convenience.  By completing this form at home, you may expedite the process of seeking help with your case.  Once the form is completed, you must still bring the form to our office IN PERSON. 

Casework Authorization Sheets will not be accepted via fax or email.  During your visit, we will also determine if you need to be referred to another office.
